The airspeed split was identified at the routine check at 80 KTS. The take-off should have been rejected at that point. The decision to continue sealed their fate.

When all else fails, fly know power settings. Not sure if they were equipped with a wet standby compass, but you can use that as well. Also, if you have a flight data APP such as Foreflight on an Ipad, use that. I had to use just that when IFR in a snowstorm going into Tokyo, for about 5mins, until we got one side up and running again with our system. We also, though, had standby GYRO we were using as well.

Whose gage is correct? Well, as a pilot you do have some understanding of your V1. What gauge is moving at a predictable rate to reach VI in the same time frame as all the other takeoffs for that plane. For example, if always before you reached V1 in 35 seconds, you ask yourself what gauge will read close to Bi in that time. Is the needle speed normal for the acceleration down the runway? The most accurate gauge is the one you trust. If you have audio on V1 then look at the gauge and check if the air speed is what is required for becoming airborne. The gauge that is way off should be considered broken and never used again for that flight. Evidently in this tragedy it read the Captains air speed gauge that was broken, so why would you turn on the auto pilot that takes input from the gauge that is broken. The common theme with these air disasters is that if anything is not quite right the automatic pilot makes it even worse. Turn the damn thing off and do your job - fly the air plane! In this scenario the auto pilot should have never shut itself off, the pilot should never have turned it on since it would be taking input from the broken gauge. The Cardinal, rule should be to never allow the auto pilot to be a part of the equation when ever a problem arises.

I know I'm an armchair pilot but speed indicator is quite a big thing in planes, should've immediately aborted the takeoff, bet he would if he could do it all over again.

This leaves me with questions. CRM not being followed and why in the world weren’t the Co Pilot and relief Capt screaming your stalling your stalling until they get an acknowledgement. I guess it just happened too quickly?

Im no expert BUT common sense would tell me that if the instruments are giving faulty/inaccurate readings, then these inconsistencies will cause issues with autopilot. Humans might have big brains but we without a doubt we have evolved to be the stupidest species on the plant

I don't understand why haven't the designers of autopilots, programed the autopilot to notify the pilots of the disagreements of information being fed into it and just shut it down and switch it over to alternate law. That would at least give the pilots a fighting chance. Leaving it active just make the pilots inputs irrelevant or worse if they fight the autopilot. To my mind I trust the pilot's ability to fly more than a glorified toaster oven

As far as I know, all planes have at least 2 ADIRUs and PITOT tubes on each side for both flghtdeck crew. So, captain should've connected his instruments to PITOT that sends data to F/O's instruments, by using the selector on the overhead panel. He had indications that the PITOT tubes which sends data to his instruments wasn't working properly, even when they were on takeoff roll. They've died for nothing.

these systems need a gps factor for backup and verifications . boeing should have implemented a system such as this long ago . why is it taking so long to avoid these error readings causing problems .. we all know sensors electrics can fail .. they need more redundancy and add GPS to the system to avoid this.. really its long over due . same with the air france crash over the ocean .. could have been avoided

Flight 603 ...The captain made 3 basic errors! First he should of aborted takeoff before V1 after noticing his speed indicator was not working,..That's the whole point of the decision V speeds! The second mistake was, he should of kept the visual reference of Lima and the city lights, instead of wandering off over a black sky and ocean, then loosing it! Then the third was not trusting the stall warning stick shaker and ignoring it, whilst still believing the instrument reading they knew were messed up! Basic common sense went out of the window on this flight, whilst they were too busy looking at just instruments. Pilot training should of covered events like this, ie in the event of erroneous instrument readings, don't leave the relative safety of a lit city! .... but obviously not. Most pilots thankfully never encounter any issues like this during decades of flying. As for sloppy ground crews leaving tape over sensitive sensors, and not covering pitot tubes during storage...Its just sad knowing so many lost their lives due to human error once again. Pilots are supposed to be extremely well trained and the best of the best...But that's obviously not always the case! One interesting note...IF future aviation does away with pilots and uses AI to fly aircraft (which it will eventually)...I wonder how an AI pilot would or could of reacted? Probably better than most real pilots! Am thinking it would of aborted the take off the instant it knew the instruments were playing up!

That captain, what a - r e t a r d, he first sees there's a problem with the air speed gauge so he knows it's faulty by asking the first officer to read off his gauge instead. So the captain clearly knows the issue. Then sets auto pilot on which is damn obvious it will read the faulty gauge and yet he seems to have no idea why the issue is caused. I don't get the complete absurd stupidity in that fact.

There is a pilot out there who is sabotaging aircraft, he has not been caught yet, this pilot is twisted in his actions. I suggest someone looks into pilots who have a prior history of being in hangers he shouldn’t have been in.

I would also suggest to all pilots to make sure to do research on whoever is keeping maintenance on the plains before signing over their lives. Too many stories have been covered up to look like accidents when really they weren’t, Smart pilots will realize the truth.

if you know at take off speed indicator is bad, then why engage Auto Pilot or even take off in first place? did captain know from audio logs it was bad and communicate it? if captain was unfamiliar with plane type, why dint rest of the crew not know not to engage autopilot which uses that faulty pivot tube.

I think that term is thrown around too much. "Pilot error" it's easy to blame the dead. "pilot error" shouldnt be used unless there is direct negligence or an unauthorized procedure.

Yes, there was a mechanical issue. But that alone did not cause this catastrophe. We have seen this before : conflict between automated aircraft control and manual control from flight crew. Opinion: this highly experienced pilot, observing SEVERAL times that there were electronic problems, should have used his experience and intuition towards IGNORING the speed indication, used his own judgement and gently nursed this aircraft back to the airport. I am VERY surprised that he did not do so. Fatigued ? Distracted by personal issues ?We'll never know.

Just flew last month to Maui. We were upset that our plan had operational problems and we would leave in a different plane at 11 instead of 7. After seeing this, I’m so thankful.
