Great podcast Vivek and Vinay. GME has gone a bad way, and we must right the ship.

Milton Friedman demonstrated conclusively that the FDA causes more harm than good over four DECADES ago in his book "Free to Choose", and I am certain in writings previous to that great book. It is really surprising to me that we're rehashing the same arguments people had in the 70s and 80s, that would have done so much good had only they been implemented by a competent government (an oxymoron I know). That said, I am glad that we're having these same conversations and that they're at the forefront of the MAGA movement, but I'm disappointed that we had to wait this long for a mainstream political movement to talk about and potentially implement this.

Lets talk about school vouchers, sound money, and abolishing the welfare state next.

Thank you Mr. Ramaswamy and Dr. Prasad for shedding light on this.

Fantastic discussion by two of the most knowledgeable people currently in our time. This reminds me of the drug Oraflex marketed by Lilly that was a monumental failure of the FDA when I was still seeing patients. Oraflex was on the market in Europe and touted as a drug for osteoarthritis that was even pushed as a drug that could reverse the changes of osteoarthritis. The FDA did not do any studies but accepted the studies from Europe. Once marketed here in the USA with the much larger patient market suddenly appeared cases of fulminant liver inflammation/failure with some patient deaths. The drug was pulled from the market and Lilly was sued by the families of affected patients. I had patients who were asking to be put on the drug but I told them to wait since was a newly released drug. The FDA is not infallible.

This convo is great. Love Vinay but he has never really addressed the excess death issue in working age people since the vaccine rollout. Ed Dowd’s work has yet to be seriously addressed or refuted.

I believe it is unethical to advertise ethical drugs to the public. The public does not understand the severity of what can happen adversely with these drugs. It also coerces physicians to prescribe these drugs or risk losing the patient to a doctor who with bend to the patient's demand. Most other western countries do not allow pharmaceutical companies to advertise to the non-medical population. As late as 1985, it was not allowed in the USA. As an internal medicine physician for 35 years, I find this advertising strategy appalling. Furthermore, The millions of dollars spent on advertising drugs like biologics every 2 minutes on TV, could be better spent on research costs so prescription drug costs could be lower.

The FDA is a big part of the problem. Why? Because they indeed are the one's who make the decisions for the US and for our government. Who gave them so much power? The FDA was said to have been formed before WWI, but it has gone through so many changes and iterations that it does not resemble its original making. The entity is not voted in. It is appointed, like so many bureaucracies in power. It is disgusting to me. As far as I am concerned they have morphed into a dangerous entity that has runaway power and control like few agencies do.

It's true that corporate capture of a regulatory agency is bad, but the way to fix it is not to disband it and rely on the free market to replace it with JD power and associates type of agencies, which would be completely free to be just as corrupt. At least the regulatory agencies have some nominal responsibility to the voters. We need to hold them accountable, not give their responsibilities to someone with no accountability.

Vivek, during the final quarter of your interview you delved into the “sheep culture” vs the lions of innovation.

As you know, the same culture shift has afflicted the Legal Profession, perhaps to different affect. Notably, the “medical sheep herd mentality” results from “risk mitigation” bought about by a century long era of lawsuits shifting burdens to deep pockets.

So many hidden societal costs in terms of insurance, product unavailability, red tape inefficiencies, and blockades against right to try; most of those costs can be blamed on the General Counsel’s office advising the Administrator’s office to avoid litigation red flags at all costs.

So the reforms once again require a cultural shift from “caveat venditor” back to “caveat emptor”.

To accomplish these reforms is going to require full on “contempt of court”, contempt of unauthorized practice rules, contempt of the self-regulating professional certifications etc. Otherwise, the cubical-farm class of “experts” who fight to the death by a thousand paper-cuts will not go quietly.
