When you have a high medicine skill you should have new skills unlock or even have negative debuffs removed like thin skin. Maybe learn how to make disinfectant, or alcohol? But to grind the stat, go bare foot in glass, and have some tweezers to pull out glass on repeat.

Isn't high level first aid make you splinting better? (In case you are drunk driver that crash every 2-3 day :v)

If for some god forsaken reason you want to grind first aid, just stand on glass barefoot with a pair of tweezers. You don't actually have to walk around to put shards of glass in your foot, and when you take a piece of glass out another one will immeadately take its place. Your left/right foot will lose all limb health so your overall health will stop going down around 55% health meaning only the bleeding can kill you but its not on a critical area like the neck so as long as you don't afk for 50 hours, you won't bleed out
